i modded my ps2 and made a custom guitar hero mod disk. but when i swap the mod disk in it shows the logos and company names like harmonics and red octane or something then the screen turns black and the disk keeps spining. what did i do wrong

i havent done anything to it. i updated a couple weeks ago but not the one you have to pay for. i have music and all that stuff on it. i tried watching the youtube videos but they lose me. can anyone give me really simple instructions on how to do this all?
